Free Vision Therapy Games at Home: What the Trials Found

The most widely recommended free vision therapy exercise in the world has been tested against a placebo in a randomized trial, and it lost. Pencil push-ups, the one where you bring a pencil toward your nose and try to keep it single, were handed to parents by optometrists for decades as the standard home treatment for convergence insufficiency, and when the Convergence Insufficiency Treatment Trial finally put them up against fake therapy in 2008, the two groups improved by about the same amount.
That is not an argument against training your eyes at home. It is an argument for being specific about what you are doing and why, because "vision therapy games" covers everything from a piece of string with beads on it to a dichoptic video game that has been through an FDA clearance process, and the evidence for those things is not remotely the same.
What follows is a list of the free vision therapy exercises and apps that actually exist, what each one is supposed to do, and what happened when researchers checked. WideEye sells a product in this category, so read the recommendation at the end with the appropriate suspicion.
The short version. The free exercises that appear in programs that worked in trials are the Brock string, vergence drills, and near-far focusing; the 20-20-20 rule is the free option for screen strain. First, what "vision therapy" is trying to change#
Most home exercises target one of three things, and it helps to know which one you are working on before choosing an exercise.
The first is eye teaming, the ability of the two eyes to point at the same spot and hold it there. Convergence insufficiency is the common failure here: the eyes drift outward when you read, the brain fights to pull them back, and the result is headaches, doubled or swimming text, and losing your place on the page.
The second is suppression. In amblyopia, the brain has learned to turn down the signal from one eye, usually because that eye was sending a blurrier or misaligned image during early development. The eye is generally fine; the cortex stopped listening to it. Exercises for lazy eye are almost all attempts to get the brain to reopen that channel.
The third is focusing and fatigue, the accommodation system that changes the shape of the lens for near work, and the strain that comes from holding it in one position for hours. This is the digital eye strain bucket, and it is the one where "exercises" most often means rest rather than training.
An exercise that helps one of these can do nothing for the others, which is why a lazy eye app is not a fix for screen strain and a 20-20-20 timer is not a lazy eye treatment.
The free exercises, and what the evidence says#
Pencil push-ups (convergence)#
Hold a pencil at arm's length, focus on the tip, bring it slowly toward your nose while keeping it single, stop when it doubles, repeat. It costs nothing and takes five minutes.
The CITT, a multi-center trial of 221 children aged 9 to 17 with symptomatic convergence insufficiency, randomized participants to home pencil push-ups, home computer therapy plus push-ups, office-based therapy with home reinforcement, or office-based placebo. Office-based therapy was clearly the most effective at 12 weeks. Home pencil push-ups alone were not significantly better than the placebo group.
The likely explanation is boring: people do not do them. Pencil push-ups are tedious, there is no feedback on whether you are doing them correctly, and adherence in home-exercise arms of vision trials is consistently poor. The mechanism is fine. The delivery is the problem.
Brock string (convergence and suppression)#
A length of string with three beads, one end tied to a doorknob, the other held to your nose. You look at each bead in turn and check whether the string appears to form an X through it, which tells you both eyes are pointed at it. If one strand of the X is missing, that eye is being suppressed.
You can make one for under a dollar. It is used in nearly every office-based vergence program, including the CITT protocol, and it has a property most free exercises lack: it gives you immediate feedback. You can see whether the exercise is working in the moment. There is no trial of Brock string alone, because nobody has funded one, but it is the home reinforcement component of the therapy that did win in the CITT.
Near-far focusing and the Hart chart (accommodation)#
Tape a letter chart to a wall, hold a small card of letters in your hand, and alternate reading a line from each, forcing the focusing system to jump between distances. Variants of this appear in the CITT accommodative protocol as well.
The evidence for accommodative exercises specifically is thinner than for vergence, and the honest summary is that they are plausible, cheap, and included in programs that work, without having been isolated and tested on their own.
The 20-20-20 rule (eye strain)#
Every 20 minutes, look at something 20 feet away for 20 seconds. This is the one free intervention that nearly every source agrees on, and it is worth being clear that it is not an exercise at all. It is a break. The mechanism is that sustained near focus and reduced blink rate are what cause most screen-related discomfort, and interrupting them helps.
Direct trial evidence is limited and mixed, with at least one small study finding that the rule reduced symptom scores over a couple of weeks and others finding little difference, but the intervention costs nothing and has no downside, so the standard of evidence it needs to clear is low.
Free lazy eye apps and browser games (suppression)#
This is where most of the search traffic goes, and where the evidence is most interesting.
The idea behind dichoptic games is elegant. Show the two eyes different parts of the same game, with the strong eye's image dimmed and the weak eye's image at full contrast, so the brain has to use both to play. Red-green anaglyph glasses, which cost a few dollars, are enough to split the image on an ordinary screen. Hess and colleagues showed in 2013 that this kind of balanced binocular presentation reduced suppression in adults where patching did not.
Then the large trials happened. The Pediatric Eye Disease Investigator Group ran a randomized trial published in JAMA Ophthalmology in 2016 comparing a binocular iPad game to part-time patching in children aged 5 to 12. At 16 weeks the game group had improved by about 1.05 lines of acuity and the patching group by about 1.35, so the game did not beat the established treatment, and in children who had already been treated, it did worse. The number that explains the result is adherence: only about 22 percent of the children in the game group completed more than three quarters of the prescribed play time. A follow-on trial in 13 to 16 year olds found something similar.
There are two ways to read that. One is that dichoptic games do not work as well as their mechanism suggests. The other is that a game children will not play is not a treatment, whatever its mechanism, and the failure was a design failure. Later work with more engaging content, including the Dig Rush trial and the dichoptic video approach that became Luminopia, a prescription device the FDA cleared in 2021, produced better results, which points toward the second reading without settling it.
For someone searching for a free lazy eye app, the practical implication is that the specific app matters less than whether you will actually use it for 30 to 60 minutes a day, for weeks, without being told to. Most free apps in the store are unstudied. That does not mean they are useless, it means nobody has checked, and a 2025 meta-analysis pooling dichoptic and monocular training studies found that total training duration was one of the two variables that predicted who improved. The other was starting acuity. Age was not on the list.
What the free options cannot do#
None of the exercises above can tell you what you have. Convergence insufficiency, amblyopia, an outdated prescription, and an uncorrected astigmatism can all produce headaches and blurry near vision, and the exercise that helps one is irrelevant to the others. An adult who has spent three months on pencil push-ups for what turns out to be a prescription problem has lost three months.
A comprehensive exam, ideally with an optometrist who works in binocular vision rather than a quick refraction, is the free-adjacent step that should come before any of this, and it is the step that the "5 free games" articles usually skip. Ask specifically about convergence, about whether there is measurable stereopsis, and about what type of amblyopia is present if any. The exercises make more sense once you know which system you are training.
The other thing free exercises cannot do is the thing that decided the CITT and the PEDIG trials: make you keep doing them. Every home arm of every vision trial has had a compliance problem, and the interventions that worked were the ones with a person, or a game, or a system checking that the work got done.
Which at-home option should you pick#
Reading the trials as a whole, the ranking is clearer than the marketing suggests.
If you have a diagnosed condition and can get to a vision therapist, office-based therapy with home reinforcement is the option with the strongest evidence, and nothing else on this list competes with it. If a clinician has prescribed Luminopia or a similar device, use that.
If you are training at home without a clinician, which is most people searching this phrase, the decision comes down to one variable, because the trials say it is the one that decides outcomes: which option will you still be doing in week six?
A free exercise sheet (pencil push-ups, Brock string, Hart chart) costs nothing and gives you no feedback, no difficulty adjustment, no record of what you did, and nothing pulling you back tomorrow. In the CITT, this arm performed about as well as placebo.
Free app-store lazy eye apps are mostly unstudied, and the ones that have been studied in the form of a plain binocular game had the 22 percent adherence problem. Many are also a single exercise, so they cover one of the three systems above and ignore the other two.

Are there free vision therapy exercises that actually work?#
The exercises used in the programs that worked in trials, including the Brock string and vergence and accommodative drills, are free and can be done at home. What did not work in trials was doing them unsupervised with nothing tracking adherence; home pencil push-ups alone were not better than placebo in the CITT. The exercise matters less than whether you keep doing it.
What is the best free lazy eye app?#
No free consumer lazy eye app has been tested in a randomized trial. The best-studied binocular game approaches were run by PEDIG, and their results were limited by adherence, with only about a fifth of children completing most of the prescribed time. Choose whichever app you will actually open every day for weeks; if that is none of them, that is the real answer to the question.
Can you do vision therapy at home without an optometrist?#
You can do vision training exercises at home, and the ones above are the standard ones. Vision therapy in the clinical sense is a service delivered by an optometrist, and office-based therapy is the option with the strongest evidence. An eye exam should come first either way, because several unrelated conditions cause the same symptoms.
How long do home vision exercises take to work?#
In the CITT, office-based therapy ran 12 weeks. In the PEDIG binocular game trials, assessments were at 16 weeks. The 2025 meta-analysis found total training duration predicted improvement. Plan for weeks of daily practice, not days.
